Fault trees are constructed to understand how a chain of events can lead to an undesired outcome.

A fault tree is constructed with the outcome (an incident, accident or release scenario) as the top event in the tree. The tree is comprised of basic events, sub-events and logic gates that bind the components together and determine the top event failure rate. These tree components are depicted in the following symbol table:

Fault Tree Properties;

  • they breakdown an event into its contributing causes

  • the tree is a combination of failures and sub-events that are sufficiently defined to yield the top event

  • a cut set is a set of primary events or unrealized events which can give rise to the top event

  • a minimum cut set does not contain another cut set

  • the complete set of minimum cut sets represents the principal fault modes that produce the top event

  • Fault Tree Example: Vessel grounding event
